During your visit, you can expect a comprehensive examination to assess your fitness for duty. This typically includes measuring vital signs such as blood pressure, pulse, and temperature, as well as a urinalysis to check for underlying health conditions. To ensure a smooth process, remember to bring along essential documents such as a list of current medications, your medical history, and for diabetics, a recent Hemoglobin A1C test result within the last 90 days or a clearance letter from your primary care physician. The Federal Motor Carrier Safety Administration (FMCSA) guidelines also highlight the importance of addressing issues such as smoking, obesity, and hypertension during the examination. Our practice utilizes Certedrive, a cutting-edge platform, streamlining the DOT physical process. Drivers must complete their paperwork on their cell phone or our provided iPad during the visit using a registration code provided at check-in. Payment is due at registration, with a consent form acknowledging adherence to FMCSA guidelines. No refunds are issued for failed tests, although our providers make every effort to work with you through any concerns that may arise. Please note: Omitting or falsifying medical history or medication history will automatically disqualify the driver.  Drivers receive their FMCSA DOT medical certificate and wallet card before departure, with an electronic copy sent to FMCSA. It is important to renew your DOT medical card at least 45 days prior to expiration date in the event you need time to have additional testing done or have forms prepared. If it is necessary to have a testing done, we can provide this service, however this will need to be a separate appointment.

Common Driver Questions

What is the effect on driver certification based on FMCSA hypertension stages?

Answer:

A driver with a diagnosis of hypertension on treatment should have at least an annual certification.

A CMV driver with a Blood pressure (BP) 140/90 may be certified for 2 years.
First time BP elevated:

  • Stage 1 - BP 140-159/90-99 Certification Period 1 year

  • Stage 2 - BP 160-179/100-109 Certification Period 3 months as one time certification. Within the 3 months, if the blood pressure is below 140/90, the driver may receive 1 year certification.

  • Stage 3 - BP Reading >180/110 Disqualified. When the blood pressure is less than 140/90, the driver can be certified at 6 month intervals.

Can I pass DOT if I am a smoker or are considered overweight?

Answer:

According to FMCSA guidelines, additional requirements are outlined for smokers and obese drivers to ensure safety on the roads. Smokers (generally over age 35) and obese individuals  (neck circumference >/=17" for males, 15" for females) or (BMI >33 with other health conditions or BMI >40) without comorbidities may face heightened scrutiny during DOT physical examinations due to the potential health risks associated with these conditions. Medical examiners may require additional testing, such as lung function tests (PFT) for smokers to assess respiratory health and sleep apnea assessments for obese drivers to evaluate potential sleep disorders. These measures are implemented to ensure that drivers maintain optimal health and fitness for duty, promoting safety for themselves and other road users.

Can I pass a DOT physical if I am prescribed Medical Marijuana and carry a Medical Card?

 Answer:

Even if a licensed medical practitioner has prescribed or recommended it, marijuana use is a DOT disqualifying medical condition. This is true whether it is used alone, as CBD oil, or in any product or preparation derived from hemp or cannabis. The federal Drug Enforcement Agency (DEA) prohibits use of Schedule I substances, which include marijuana, heroin, LSD, mescaline, MDMA (“ecstasy”), psilocybin mushroom, methaqualone (Quaalude), cathinone/khat, and 3,4-methylenedioxypyrovalerone (MDPV).
